(1) Copyright registration application form;
(2) Applicant's identity;
(3) Proof of ownership of rights;
(4) Sample of works (can submit a paper medium or electronic medium work samples);
(5) Work description (from the creative intent, the creative process, originality to lay out, and signed by the author);
(6) Power of attorney;
